We are committed to providing our patients with the finest possible care and service. Your comfort, well-being, and satisfaction are always our top priorities.
Billing
Please come to your appointments with all necessary insurance forms and cards, so that we are able to bill your carrier in a timely and accurate manner.
Patients can pay with cash, check or credit card (VISA, MasterCard, Discover or American Express). Patients that do not have insurance will receive a cash pay discount and will have to pay at the time if service.

Insurance
We accept virtually all insurances. Please contact our office at 360.578.1188 if you would like to confirm your coverage.
Payment
We accept cash, checks, VISA, Discover, American Express, and MasterCard. Regardless of the amount your insurance company pays, you are responsible for full payment of all bills incurred at Longview Physical & Sports Therapy.

What You Need for Your First Appointment
Please bring the physical therapy prescription from your primary care physician, as well as your insurance card. Patients should bring claim info if they are being treated as the result of a work injury or motor vehicle injury.
Clients receiving shoulder treatment should bring a tank top, sleeveless shirt, or blouse. Those receiving lower extremity care should bring shorts or loose fitting pants.
To help save time, we also recommend that you download, print and complete the applicable forms (see “Patient forms“) in our Patient Info section.
