What to Expect on your First Appointment.

Here is what you should expect on your first visit. When you first arrive for your visit, your temperature will be taken at the door. Please come 15 minutes early and be prepared to fill out some paperwork by having the following with you: 

 

  • 1. A photo ID/Driver’s License (the parent or legal guardian, if the patient is a minor) and your insurance card(s).
  • 2. Any copayments, deductibles or coinsurance amounts as required by your insurance company.
  • 3. If you are taking multiple medications and have a printed medication list, please bring and we will scan it into the system for your convenience and ours.
  • 4. Wear comfortable and loose fitting clothes, especially around the problem area to be examined and treated.
  • 5. If desired, one person may accompany you to your visit but no more than one.
  • 6. If you are a minor, a parent or legal guardian must accompany you to help complete paperwork.

Please have all of the above items with you when you arrive or it will be necessary to reschedule your appointment. After the evaluation has been completed, the therapist will discuss with you a treatment program and plan for a next visit. 

If for any reason you are unable to make your appointment or need to reschedule, it is very important that you call our office (preferably) 24 hours in advance.

If you are showing symptoms of Covid-19 or have been recently exposed to or in recent contact with someone who has contracted Covid-19 please call to let us know and we will be glad to reschedule your appointment.
